Just my two cents, I wear both for many years. The blood escapes from the dorsal side of the shaft (the top side -facing up while straight erect). The purpose of the c-ring is to press down and prevent the blood from escaping at the base of that top side. What’s below that portion doesn’t really do much except maintains that tightness so that the top applies pressure. I personally prefer ones that go around the shaft because sometimes wearing the ones around the balls can cut circulation to some pretty narrow veins and can damage them if worn for a few hours. Plus it can get uncomfortable after an hour or so. I find the ones around the shaft more comfortable and keeps the erection but everyone has their preference. The ones I use look like bigger stiff rubber o-rings (non-stretchable) that have a diameter for it to stay on loosely while soft, and then cut blood from leaving via the doral vein when I am hard.

If you are wearing it for presentation (example nudist places or beaches), the ones that goes around the balls has the old push-up bra function where it lifts it and makes it more presentable. You give up the longer dangling look but makes it more prominent looking. I use the shaft one for those occasions so I have the longer dangling look but it still hold a bit of blood in there to make it look half inflated (a semi).
