@Dr PickleSorry for the late reply,. I can break the concepts down more based on what Clive Barker originally stated that the Cenobites were human like beings in an extra-dimensional reality rather than a Hellish Afterlife type world.

So basically all the mutilations are a mix of body augmentation and implants.
The Leviathan is a Super Computer that all these beings are linked into.
The majority of Cenobites cannot distinguish any longer between their own augmented reality and distinct physical existence.
The few that "rebel" are those that became aware they are all trapped in a hive mind their own species created a very long time ago.
The Leviathan then being an AGI (Advanced General Intelligence) gives reference to itself in terms it can best equate with what it learns from the minds it is linked into (such as it comparing itself to the god/master controller of that world).

If we really go back to the very first we see more of this being the case and later the various studios choosing to :redefine things in terms of Hell and demons and the Devil more or less pushing the narrative movie goers at the time wouldn't understand it otherwise. One has to remember that the concepts of actually achieving some sort of virtual reality and augmented reality was already in place in 1987 so a lot of movies at the time dealing with ethics, morality, fear, fantasy and tech were some of the most common themes. Even Cartoons of the time were experimenting with such things.

All the Labyrinth is, is a representation of the "core mind" of the Leviathan and the things in it the various minds linked into it.

I always thought that Leviathan was the coolest thing in the entire Hellraiser franchise. How it towers ominously over the labyrinth, the sound it makes, and getting pierced by its black lights. It was so intimidating and almost Lovecraftian.

Leviathan's geometric "body" apparently has "rooms" in it, according to some of the comics.
TBH, I always imagined Leviathan's true form, as the name implies, to be some immense aquatic beast, housed in an immense holding take at the centre of that geometric shape, the last of its kind from a dead universe, with near omnipotent psychic and psychokinetic power within its own realm.
