Lost Time Memory features the Shintaro of two different story routes; One who is able to accept that Ayano has died, and one who is unable to, who is shown during scenes of Route XX.
The PV starts off with Ayano and Shintaro walking down a bridge and him telling her to leave him alone. Ayano, however, refuses to leave his side, so he calls her annoying, then walks ahead of her.
Following, Shintaro is seen in his room, the time and date being August 15, 12:33. He decides to go outside, bringing Ene along with him. Wishing to die already, he clutches his wrist, and steps in front of a door with the numbers 107 on it. The door is opened and in front of him the other members of the Mekakushi Dan appear. He greets all of them, shortly after hearing Ayano's voice, which suggests to "play" once more.
The events of Route XX begin with Ene stating she is worried. Shintaro starts to wander all about his room and finally drifts to sleep. He dreams about Ayano and him in their middle school years, until he hears Ene's voice asking if that means he cannot even see tomorrow. Promptly, he opens his eyes and turns to strangle Ene, who disappears from his grip, seemingly having died. Shintaro suffers a nervous breakdown and takes out a pair of scissors.
Another scene from Route XX begins to play, proceeding from when the Snake of Clearing Eyes takes over the body of Konoha, therefore turning into 'Black Konoha', in order to kill the Mekakushi Dan and repeat the tragedy they are part of.
Route XX Shintaro sits on his bed and stabs himself with the red scissors, while in the parallel scene, Black Konoha attempts to shoot himself with a gun. However, Shintaro takes the shot, causing himself to die differently in both scenes.
The next scene shows Route XX Shintaro and the other Shintaro lying on the ground, surrounded by a white nothingness. Route XX Shintaro sits down, with Azami looking down at him, until the other Shintaro walks past her, much to the gorgon's surprise. Route XX Shintaro and him appear to be in an empty classroom, back to back. Ayano stands in front of Route XX Shintaro and apologizes for dying, whereupon she asks if they should say their farewells to each other. Shintaro runs after her, begging her to stay, again expressing that the Shintaro of this route has not yet accepted her death. The other Shintaro walks forward and smiles at a surprised Ayano, who smiles back at him. She then wraps her scarf around her friend, whose eyes turn red, proving he has accepted Ayano and gained an eye ability.
At the end, Route XX Shintaro is seen lying down on his bed, the date and time being August 15, 12:32.

The MV is filled with parallels, but there's a few I've noticed that I haven't seen many people comment on.
"ikanai yo" - "I won't leave you" from Ayano at the beginning as she reaches for Shintarou's hand, does a reversal at the end with XX Shintarou as he yells out "ikanaide" - "Don't leave me". This time it's Shintarou reaching out for Ayano.
Throughout the MV it brings up"kageboushi" the "silhouettes" or the shadows left behind by Shintarou and Ayano on that sunny day. The idea is that those "shadows" as in, the memory of Shintarou and Ayano, the kinds of people they were back in that exact moment in the past, still exist in some metaphysical way, as shadows, which are continuing to watch the Shintarou of the "now".
The beginning scene ends with Ayano questioning whether or not that Shintarou was the real "him" or rather, was that really the way Shintarou wanted to act, and the things he wanted to say that day? The MV never explicitly says but Shintarou most likely feels the same way. A part of his trauma comes from the fact that their final moment together ended in such a negative way or rather, in a way that was unlike him.
The final line of the MV, "the silhouettes were watching this kind of me", is him declaring that the shadows of both the Shintarou and Ayano of the past, that have continued to exist alongside Shintarou all the way up until this moment have been watching and waiting, for the "real" Shintarou to make his return one day. It's been a long time coming, but they both finally get to see this kind of me again.
The whole concept of versions of ourselves existing as shadows and watching the present us is metaphor to Shintarou's hikikomori ways, the same way he can't get over his trauma and is stuck in the past and can't move forward, there are shadow-selves which were paused at a specific moment in time, waiting for the moment Shintarou's time starts moving again.
There's a lot of Japanese media that play with the "in that moment, my time had stopped", that sort of stuck in the past, unable to move forward sort of thing, but this is by far my favorite portrayal of that idea, due to all the parallels, metaphors, and layers to the entire story.

This is just my attempt to explain the story of this song with the info from manga and the anime, as this give a new way to interpet how the Route unfold on Shintaro, but the main focus here is Route XX. Spoiler alert, and this is just an interpetation.
As shown in the MV itself, Shintaro always ended up reunite with Ayano in Kagerou Daze after his death, whether he actively reach for the classroom (Route 1) or no (Route XX), so it's safe to assume that this happen at the end of every Route as well. The beginning of anime likely hinted toward this as well, as the scene after the ShinAya convo possibly is Marry rewind time, making Shintaro reunite with Ayano the last thing happen in a Route, after Clearing Eye Snake slaughtered the Dan. Later in the anime though, Ayano turned out to be Shintaro's Retaining Eye Snake and her attempt to regain the memories Shintaro lost in time. Manga further confirm this is what happen at the end of a Route, when Shintaro reunite with Ayano and she relay to him the Route when she became his snake to retain his memories. Which means, the ShinAya reunion within the MV possibly isn't Shintaro reunite with the Ayano trapped in Kagerou Daze, but Shintaro with his eye power.
That made me interpet Route XX differently, even though the core is still about the Route when Shintaro could not accept the death of Ayano. Adding in the information from the anime, that Retaining Eye power work passively for Shintaro giving him a perfect memory, it is possible that the perfect memory is the major factor drown him in the past with memories of him and Ayano, and is what make him cling to those memories tightly, afraid he would lose it. This is different from Route 1 when meeting the Dan maybe have distracted him from the negative effect of perfect memory, but for Route XX Shintaro relied on it to relive his memories of Ayano. Said remembering ability is the passive effect of possessing Retaining Eye (and Retaining Eye is Ayano, time loop shennanigan), thus beside Anime Route, Route XX Shintaro maybe is the Shintaro who could trigger his eye power before the loop happen (anime suggested this which a scene from Shintaro in Route XX hoodie awake with his power triggered, manga also suggest this as Shintaro awoken in said hoodie with Retaining Eye triggered after the rewind). That could be what happen after Shintaro killing Ene, as in 2:38 he sitting and clutching his head similar to the anime version of Lost Time Memory. He not only crushed by the guilt of killing Ene, but also by remembering everything happened in previous cycles. Possibly, Retaining Eye clear out the confusion afterward, but realized the grave mistake he made, Shintaro choose to end his life, which is why he is so give up in 3:38, and reunite with Retaining Eye Ayano, seeing her bid him farewell wreck him completely.
The reunion in Route 1, in the other hand, have Shintaro finally come term with Ayano's death and accept Retaining Eye Ayano's farewell, and even though the Dan has lost to Clearing Eye Snake/Kuroha, he bid farewell to Ayano and face the next cycle with a smile. Retaining Eye Ayano responed with a smile in return as she wrap her scarf around Shintaro, and this is different than anime version when she crying while time rewind by Marry. Ayano in the MV disappeared after that could suggest that Ayano is the snake one rather than the other Ayano too, and after that Shintaro's power triggered, which the MV shifted back to Route XX Shintarou lying on his bed.
TL;DR:
Route XX: Shintaro activated Retaining Eye after killing Ene, completely lose all hope and kill himself (both by the guilt of killing Ene and realized that he fucked up massively)
Route 1: Shintaro activated Retaining Eye right before time rewind, with the will to fight and reach the future that carried on to the next loop, when he would activate his eye again before the Tragedy happen and stop it.

For those who are confused about the two routes shown in the video:
(First things first you need to know the girl with the red scarf is Ayano Shintaro's she committed suicide which caused Shintaro (in any route) to go into depression and become a shut in)
-First we see Shintaro with the red jacket and is the Shintaro of route 1 (manga), he went outside and met mekakushi dan
-Then we see Shintaro waking up with the black hoodie he is the Shintaro of route XX (song) couldn't let go of Ayano and killed Ene (the cyber girl)
-In the next scene is the Shintaro of route 1 with the Mekakushi Dan facing off with Kuroha, I think Kuroha was going to kill himself but Shintaro took the shot for him dying and as for route XX Shintaro he couldn't let go of his memories of Ayano (she killed herself btw) and committed suicide with a pair of scissors thus both of them lying next to each other dead
-In the next scene we see Azami (the woman) looking down at XX Shintaro and pities him because she had gone through the same experience of losing a someone close to them who finally understood them as they were avoided by everyone else, then we see her surprised as we see route 1 Shintaro walk past since he can accept the death of the one he loved's death (Ayano) and she is surprised because she never had this strength to move on
-Next we see both XX route and route 1 Shintaro in an empty classroom with Ayano as she says "I died I'm sorry" and "Shall we say our farewells" XX route Shintaro begs her to stay since he hasn't let go of her death, route 1 Shintaro instead smiles at her and she is surprised since it means he accepts her death
-She wraps her scarf around him and then his eyes glow red, this means since he accepted her death and because of this he is granted the retaining eyes (the ability is to rememberer past routes)
-The next scene shows a Shintaro with a black hoodie lying in bed you could either think of it in one of the two ways
- He is the Shintaro of route 2 (most likely)
- He is the route XX lying in bed in depression because he can't let go of his time his Ayano (might think this because he has a black hoodie)
