The low body fat and pale skin throws you off. People will usually have more tan due to sun exposure, but he's avoiding the sun to minimize skin cancer risk, and takes a vitamin D pill instead (usually your skin synthesizes vitamin D through UV light exposure from the Sun), and you associate pale skin with being sickly which is not always the case. You can just avoid the Sun to get the sickly look without actually be sickly.

Also his low body fat makes the bones on his face more pronounced , and terminally ill people lose weight and get a similar look in their face which is the other thing that throws you off. But the terminally ill people lose weight and continue to get worse since they don't eat and eventually die, while he eats and keeps his weight constant. Also you might be used in general to see people with unhealthy levels of fat which includes a fuller looking face, associating normally looking people ( "normal'' just means most people at this time, and doesn't mean that something is "good" ) with health, which is not the case.
