Depends on your way of thinking,
my Granny died at home, and when an old person slowly dies you can see the change on the face every day. "as your face falls apart"

In my life my Granny was very Importent to me, so i would really do alot to just see her again or to touch her cheek and say that i love her and that i am thankfull for everything she did for me.

So in the End, this song can give many ways of thinking.
its up to the Person who hears it, what to believe.

I really love this song, even if hearing it makes me feel so sad and Helpless.

I think I got the meaning for those who are completely lost... First part. He lost someone he dearly loved. And by saying ' you lie there helplessly still as your face falls apart' Meaning that, that particular person is dead. The long pause of music describes the work he put in, into "FIXING" That per some he loved...
