The lyrics of Mech's song Painkiller depict the singer's state of mind as he descends into a maddening state of pain and violence. The first stanza describes the intense state of mind of the singer by using the metaphor of "rivers of madness" flowing through his veins. He declares that there is no turning back now and the only thing left is the pain.
In the second stanza, the singer expresses his aggressive attitude towards the world that has caused him to suffer. He is not ready to back off, and he is ready to take on any challenge that comes his way. He is looking for shelter, but there is no place to hide. The reference of "evil helter skelter" suggests that he is surrounded by chaos and disorder, and he has to face it all alone.
The chorus of the song presents the singer's outlook on his pain. He wants to kill the pain and let death reign. I am a HUGE fan of the Painkiller series and I know the lyrics to this song.......BUT.........
back in 2004 when this was released and I didn't had an internet connection,

Bought it on steam sale recently, forgot about it, and am playing it again. As a veteran of the genre I am SHOCKED at how well it holds up.
Not everything is perfect, the boss fights are weird, some of the levels have problems with enemy spawns being sparse or timed strangely, but overall the experience is still friggin' excellent.

This was one of the first FPS games that used ragdoll physics for the enemies.... it was bananas! and playing to this soundtrack was awesome!

I'd say that it's underrated compared to today's standards. Particularly with Doom (2016) and Doom eternal. It would be nice to see Painkiller make a comeback one day with a true "Painkiller II" (not another one of those expansion packs).
