I'll try:

K'Naan starts off talking about what Africa goes through. "Drink poison, then I vomit diamonds." Probably means Africans are given false hope and promises by the people exploiting them for blood diamonds. Maybe they're actually given alcohol. Then, he starts talking about what Africa has given the world Mandela aka Black Dalai Lama and various forms of music, namely jazz, which has evolved in the Western world.

Had to look up Donny Imus to understand the next line. He was the radio host who called the Rutgers Women's basketball team "nappy headed hos". K'naan is pissed because rappers are generalized as people spewing vulgarity.

After that he starts breaking it down. "Me and my nation against the world". Likely speaking about Africa as a whole. "Me and my clan against the nation". His close people in the city sticking together. "Me and my family against the clan." Family within a city sticking together. "Me and my brother without hesitation". The people closest to you, best friends or brothers (or sisters, cousins, etc.) have your back. "Me against my brother. Pull a Kalashnikov and kill one another." Probably referring to people doing whatever it takes to survive in dire conditions. Kalashnikov invented the Ak-47, which are prevalent in Africa.

This is my loose interpretation, so others might see and hear it differently
