I've never understood how composers can create something like this. The notes sound totally different when played at tempo than when you practice it slowly. I wonder how they could know how the piece would sound with such complicated chords and notes running together but at Allegro?

I have the same thought while working on Rachmaninoff etudes. It's like an impressionist painting that we look at closely and which seems abstract to us. It is only when we take some distance that it makes sense.

I felt the exact same when practising Chopin's op 10 no 4 torrent etude.. It sounds nothings like what it's supposed to when its slow.. I asked a few pianists and I realized that composers don't compose from bottom to top but from top to bottom.. So they imagine the notes at the tempo given to the piece (because of their insane ability they can do this and know which notes are required), then they slow it down add a few details to make it better and then speed it up again!

They know how it 'sounds' without having to even hear it. Their knowledge of music theory and familiarity with the different chord combinations probably is sufficient to not have to hear it at tempo when composing.
