It's gonna be alright: He's pretty much just miserable at the moment, he's lost in his drug use and wants to get back with his true love

Pink eye: Heroin, he's trying to breathe, and his dog is barking at him to get up. He likes heroin a lot so he gets hooked on opiates for a while

It works well in a few ways but I like it most in the pirate sense - he's now embittered and rampant on the high seas, plundering other ships/women. Maybe with pain deep below...but drowning that in beast mode.

An elderly sailor, realizing his time is nearing its end, tosses the mollusk into the sea off a pier and watches his ship sail away into the fog...

...During the "I'm Dancing in the Show..." reprise: Years later, a young child stumbles upon the mollusk shell while walking along the beach. He picks it up and walks away. Just like the album/story also began.

This is what I picture. One of my favorite albums ever.
