"The world continues without you." I try to create this idea in my sandbox setting by having NPC storylines going on in the background. The major NPCs in the setting have their own goals and agendas and their own little stories happening, which I update between sessions (a brief paragraph for each NPC). If the players run into one of these NPCs and decide to interact with them they might get caught up in their story and turn it into a full-blown adventure, otherwise it's just background colour and tavern gossip
A couple of sessions ago they heard that a local thief was due to be hanged on the outskirts of town the next day, in case they wanted to go and watch. They went off to do something else, and the next day they heard the thief had made a dramatic escape from the gallows and was believed to be hiding out in a nearby valley, with a bounty on his head. They didn't follow up on that, so now the gossip says that the fugitive thief has joined up with his old gang and is robbing travellers on the highway to the south, and the bounty's increased. If the players eventually decide to hunt him down and collect the reward, it'll become a storyline in our game. If not, they'll continue to hear more and more wild gossip about the increasingly audacious exploits of this famous thief.

"Walking the world" - what a great way to describe how you create a scout map - I love the idea of adding very unique physical features and also the Mt. Rushmores of the world (man made features). I create/draw maps and they always tell a story to me as I create them. I write a map history to go with each that creates a world that DMs can populate and expand upon. I would take the opposite tack on commission maps to what Deven describes - no offense, I am extremely in awe of what she creates - I just always like the input of another mind it stimulates me to both create and innovate to fit into what they have described - so for me the more detail the better.
