One of my most favorite things about her lyrics are the hidden intentions and double entendres.
"As I'm getting thinner and more worthy of your sex" can be interpreted, of course, as the physical relationship with the man.
That said, she writes a lot about the expectations of what it means to be a woman from a lot of different angles. At least, the way I read them.
I read this as "worthy of your sex" in the terms of becoming a woman worthy of the attention of a man, in a social expectation sense. Ex. The social expectation for women to be beautiful in a very specific way. Thinness, weight, style, features, etc. (Not the way I think, btw)
Anyway, I read this song overall as the story of the woman who followed the rules, and was faithful to her man, who ultimately left her alone when their relationship succumbed to the distance built by her constant aspiration toward the "ideal femme".
I think you're absolutely right with the manipulation part. Because she's playing a role, and the world she's built requires that she is seen in a certain way. That she's perfect. There's no real depth beyond that, and so she hides her emotions and creates a gap in doing so.
I don't think she's begging him to stay because she loves him. More that she needs him to appear the way she feels she must be seen, which makes it impossible to see him as anything more to an accessory to her ambitions.
It's a really tragic song, and a cautionary tale, IMO 🤷♂️

it hurts pretty bad when she says 'you know I'm so happy,you say you're so lonely' because in my interpretation both of them are lying, she says she is happy (although the whole song portrays her suffering) just so he won't see her fragility and take advantage of it.
