Yo Derek, can you do a video on the efficacy of Kisspeptin as a replacement for HCG? Here in the US, HCG is now listed as a biologic and so most pharmacies aren't allowed to compound it anymore. HCG is what combats testicular shrinkage from TRT.

@Christ power he literally has like 10+ other videos that basically tell you all you need to know atm, which is basically hopping on the "kitchen sink approach" (using a combination of topical chemicals like RU, CB, topical bico etc) + systemic androgen reduction like finasteride or dutasteride... In a perfect world, a guy prone to mpb would hop on RU when he was still nw0-1, because prevention works way better than literally fighting an uphill battle once you get past the NW2 stage

I’ve wondered how much something like pine pollen will work to balance natural hormone levels. I think stress plays a roll too. Nutrition. Stress. Then again I’ve seen some guys that stuck a needle in their arm full of opiates for years until they almost died. They still have a head full of hair and perfect hair line at 60. Ive seen the idea that estrogen related side effects show up when the E:DHT ratio is thrown off in favor of estrogen. Some say that a low dose of a DHT derivative or straight DHT could put that balance back in order to prevent high estrogen side effects. Your thoughts?

I wonder if there's a threshold estradiol level that promotes healthy hair. Because it seems like your estradiol levels ultimately drop below a threshold level and you start to see accelerated hair loss. Furthermore, from my recollection, trans women (MtF transexuals) cease androgenic alopecia (and maybe even reverse it, but I'm not sure about that).

I have to disagree with the Youth and Fat issue from personal experience. Prior to my 25th Birthday (gonna turn 35 in a Week) I grew hair like a Chia Pet where my Head was so full of Hair that it grew out of control (during this time I weighed between 220-250) however, a couple of Months after my 25th Birthday I dropped down to 190 lbs as I exercised, dieted, and did MMA yet I looked in the Mirror one morning and for the first time ever could see Areas of my Scalp where I suffered Hair Loss as my Scalp was no longer full of Hair meaning I ironically lost Hair when I lost Weight versus when I was overweight being less physically active. I've either had to shave it to a 1 or wear a Hat to hide it as it's noticeable when it grows long on top or gets wet. I'm considering on getting prescribed to Finasteride to see if some of this DHT could be killed off so my Hair Follicles could be reactivated.

DHT isn't the full story for androgenic alopecia. There is the well known DHT paradox, DHT in the scalp correlates with thinning, but on the rest of the body it helps hair grow. In the scalp, some theories suggest DHT causes the subcutaneous fat layer to get thinner, eventually restricting capillary blood flow around the follicle. Maybe losing weight means less fat on your scalp too. You may notice in people who aren't naturally bald, but shave their head have wrinkles up there. Bald people pretty much always have tight scalps from what I've seen. There is good evidence to suggest that daily scalp massages are effective for re-growing hair.

@Bumblebee Tuna that's too bad. Did you do it for at least a few months? Maybe you had the wrong technique. I'm no doctor though. And everyone responds differently to different solutions. Hope you find something that work for you.

What I do is basically squeegee from back to front for about 10 min a day spread over a few sessions. It helps to use a few drops of oil like jojoba. Seems to have stopped shedding after a few weeks. I hope to see results after another month or two. It feels good so I would probably continue to do it once in a while even if it does nothing for my hair.

My situation is weird because I still have Hair and when I let it grow long on top it almost looks full until you look in closer to see some Parts of my Scalp. This was never the Case prior to my 25th Birthday. I will consider that advice of yours. I am getting into the Field of Regenerative Medicine because I am passionate about restoring/maintaining Health/Youth.

I have a theory but many people won't like it... there's something called the "androgen paradox" where dht and testosterone is needed for hair to grow thick and strong, but it's also "linked" to MPB... I believe the DHT is being up-regulated at the scalp because it's detecting that it's thinner, and it's attempting to thicken it... but lacking energy, nutrients, blood flow, etc... it's still going to thin out and eventually shed...
I think SOME DHT blocking medications work because they've been shown to be chemically similar to progesterone, like finasteride (that's why pregnant women always rave about how good their hair feels and looks when they're pregnant).... progesterone is also feminizing...
So blaming DHT is like blaming cholesterol for the heart attack... it's their for a purpose, but it's not the cause.

Stimulate hair growth and provide blood and nutrients i guess.
That way gene activation would be prolonged in action so instead of you losing bunch of hair in span of 2-3 years you could strech it out on 10 or more years or in mild cases for lifetime with bare change.

Even if lower conversion to Free DHT in ageing men might be the case universally, some men bald, some don't. It's noticed by multiple doctors that you get the hairline of your mother's father... Dr Rand mentioned it somewhere in a video too.

So it simply might be that some men genetically have a higher DHT response in the scalp probably because of DHT receptors in the scalp alone getting agonized due to ageing... Which is why DHT blockers work regardless of DHT lowering.

Also there might be a higher DHT response in men's vocal chords as they age as older men have deeper voices than 20 year olds.

How much estrogen do you need to have before your body signals to stop producing Testosterone? I'm asking because I'm on finasteride which increases T to E conversion and I wonder if it's enough to start the signaling. Also, if that were to happen, does that mean you cannot increase your T anymore with other methods like working out? Is your body gonna suppress further T increase or only the natural component?

So regardless of how much testosterone that we produce, we will naturally aromatize a proportionally larger amount of testosterone into estrogen? Or is the excess estrogen usually only the result of increases in body fat et al? And if so then what is the point of starting TRT at an older age? Shouldn’t the focus be on minimizing extraneous estrogen production that doesn’t involve testosterone?

One thing Derek did not mention is that when someone has low testosterone levels the body produces more 5-a reductase enzymes in the scalp for some reason. Soo even though old men have lower free DHT levels in their blood, they may have a higher local conversion of testosterone into DHT in the scalp ---> more hair loss.

If I would be "normally" a NW2 with 30 years and a NW3 with 40, but I get to NW2 already with 27 because my doctor prescribes me a DHT increasing medication like Mesterolone or TRT and then I stop it, will my hairloss slow down or catch up to the normal pattern again and I get NW3 with 40 like I would or will there be always a disadvantage and I still will be reaching it earlier?

I've always theorized that male pattern baldness is the bodys response to low T. The sun exposed scalp gives you vitamin D which is linked to T levels. The older a man gets, the more T he loses. So the androgen receptor becomes more "open", which is also the problem with prostate cancer I believe. I think it was also proven that young men with premature balding have low levels of free T

Dereck do you know Ray Peat and Danny Roddy? I studied their work a lot and basically solved my problem with alopecia lowering estrogens and prolactin, raising my progesterone, testosterone and dht and increasing my metabolic rate.

Yes. I evaluated their stances objectively in the past when I was first learning about this stuff and they simply don't hold up in any case that has to do with actual AGA. Alopecia or Telogen Effluvium is different than AGA.

Answers for your questions.
You can regrow most of your hair but have to undergo severe hormonal changes like transgending. So thats out of the question.
But what you can do is Minoxidil 10% ( or 5%) twice a day, Dutasteride 0.5mg everyday , Nizoral every other day( ketoconazole without zinc pyrithione as I found out that Zinc pyrithione in nizoral shampoo counteracts Min's effect in a study). Thats the best you can do. And add dermarolling every week or tretonin 0.025% every week(has side effects with light so use at night and wash off in the morning). Onion juice on scalp everyday( Yes, there are studies, its real) This is all you can do. If you are way beyond like NW6. I would suggest you get a hair system, the more costlier it is the more better. Trust me you wont recognise a hair system these days, its that realistic. Or shave it off, grow a beard and lift weights.

And natural remedies?? Please. 🤷🏻‍♂️ Onion juice works. There are studies on it. You can add it in the Main regime as an add on to maximize effect. Thats about it. Saw Palmetto doesn't work. All are scams.

Bald men have a calcium deposit under the scalp that's been noticed during autopsies. Perhaps the drinking of hard water just affects some men that have poorer blood flow more.

Derek maybe that is why I feel better in age 43 on my TRT with small dose of Proviron? 25-50 mg per day. I just feel younger ;)

I have a totally and completely hypothetical question, I'm not even asking for a friend lol. If I understand this topic correctly (big if) aromatase inhibitors lower estrogen in the body. The body's feedback loop senses the lower estrogen and compensates by increasing testosterone production leading to an increase of estrogen from more testosterone being converted. Is it possible to increase testosterone production to a supraphysiological level or a high end of "normal" by only using an aromatase inhibitor?

Very low dose topical Fin mixed with RU. I use just enough low dose Fin to lower scalp DHT because it has more binding affinity than T and RU. It helps make room for RU to work better as DHT isn’t in the way and it can then bind better.

Get some stinging nettle root. It binds to shbg. It maximises the free testosteron and reduces the conversion of estrogen. I have used it and i really recommend it. Helps your hair especially

I start losing my hair after i got a bilateral varicocele, my T level went from 700 ng/dL to 360 ng/dL, besides losing hair i gain fat, lost some muscle and i grow hair on my body. so i think i lost half of my T my estrogen goes up my body tried to compensate with DHT bye bye hair

@MalBoYover9000 i think you need a balanced level between T and estrogen, if you noticed the majority of top athletes have full head of hair, and i think that is due to the balance of their hormones that contribute to a better performance
