i feel like this song is about a codependent friendship or relationship in which one person has a saviour complex, and the other is someone who is traumatised/mentally ill and relies on the other person to feel okay or for stability. it's slowly breaking the "saviour" person, or the "angel", and destroying their mental health, but they just have to keep pushing with a fake smile, because no, angels don't cry.

the song is sung from the pov of the 'saviour' part of the relationship (we'll call them person A), and the other person in the relationship relies on them for security (they're gonna be referred to as person B). self explanatory lyrics - person B seems to 'require' help from person A, as they do not have any other supports, and person A cannot bear seeing person B suffer.

'save your life' could be metaphorical, as in person A helping person B to get through a hard time, or literal such as talking person B out of suicide or self-harm, or other dangerous/risky behaviours. it's not a bad thing to help a friend or partner struggling, but with the wording of 'made me do it for you every time', it has been continuous and implies that person A is not getting any other forms of help, with the 'made me' implying that person B perhaps manipulated person A into doing so, whether intentionally or not. however, person A, the 'angel', despite being stressed and negatively affected by playing as the therapist friend/partner, puts on a brave face and doesn't cry - crying would be an expression of their emotions and how they truly feel inside, and an obvious display of how poor they feel - but person A tells themselves to not do so, as they believe they must prioritise person B's wellbeing above all.
