He and his partner Jigsaw Jimmy run a gang and have a cozy business selling drugs to the Mexican Mob. The chorus reveals that "Dr. Feelgood" is a nickname for Jimmy, who is the one that makes people feel alright by selling them their drugs.
The second verse tells us more about Jimmy's operation. He has a hideaway where he does business during the day and sells sweets (candy cane) to the people on the street at night.
