The singer is reminiscing about better times but is now struggling with sickness and bad feelings. They long for the “good times” they once had but are clearly struggling in the present.
The lyrics also touch on unrequited love and the pain it can cause. The singer asks if the listener has ever loved someone who did not love them in return, and how unsatisfying and unfulfilling that experience can be. The lines “Love is a worried old heart disease/Look like the very one you be lovin, is so hard to please” is both a metaphor for love and a description of the emotional turmoil it can cause.
