It's about a husband and wife. The man has dementia, so the beginning part of the song is a mixture of random memories (which is why the song is lyrically and musically kind of crazy) then towards the end he talks about being on heaven's door--"Gideons bed" = hospital bed because Gideon bibles are generally there. and "going to winter" = dying. It's sweet because she sticks by him even though he doesn't recognize her. The dog represents the impending death/dementia taking him over.
