Magnolia marked a significant shift in the band as their music shifted to a sound reminiscent of alternative rock and grunge, unlike the pop punk songs of their previous releases. Of course the most well-coordinated-only time to Re-scrub in-turnovers I’ve ever seen, was my Ophthalmologist friend going between two OR’s cranking out 20 cataract or LASIK surgeries in 3 hours using 2 OR’s.

As a FM resident I also realized that all of those LASIKs were cash pay. Essentially, that man would make my yearly income every couple surgery days. Sigh…where are my crooked glasses?

To be completely truthful, all I remember was scrubbing between cases. It could easily have been 20/hour.
He was totally smooth with the patients in the LASIK surgery. At the end after the patient was sitting up on the table, he would ask the time. The patient would inevitably look for the clock then be ASTOUNDED that they could not only SEE the clock, but the minute lines in great detail—and all that with goo in both eyes. SMOOTH….same script every patient….he is an awesome businessman, and he took great care of my DM patients. We would bump into each other in the elevator of our building. He had the top 2 FLOORS, 3 amazing sports cars, and a couple NICE Harley’s.

He also did the Ophtho lectures at my med school, and would tell the students to never say “just the med student”—you are already in the club! And he offered everyone professional courtesy LASIK which I jumped at after being blind all my life and lusting after PRK (then LASIK) ever since it was first shown in the early 80’s on “That’s Incredible”. I used my med school loans to pay for it—only the surgical center fee, of course.
Again, smart businessman as he was known as the doctors doctor.😉.

They are your boss in the OR and unless you're a senior nurse who has worked with that surgeon a lot you have absolutely no business being disrespectful or showing visible irritation like that. As a regular complex patient I see this when there’s delays getting iVs in or other reasons, they stand and sigh and regularly ask how much longer, the PICC nurses are so used to it. There was only 2 times where a surgeon came and stood with me for a bit and actually talked to me. I felt so special and cared for and I’ll always remember one telling me he fostered seeing eye dog puppies - he really did have a warm blooded heart unlike a lot of others 😂. I felt so special it was the best 6-7mins OR minutes ever!!

In the UK a few years ago there was a German orthopod who was just leaving the NHS after some years . He had p****d off his clinical colleagues because he had worked so efficiently that the ortho waiting lists were minimal and thus fewer patients were opting to go private to jump the wait ( a feature of the NHS from the start which encouraged inefficiency ) . Apparently he said that he introduced the German system where he used to use two theatres and when he was finishing one patient then the next patient was being prepared in the other . So he would work with minimal turnaround time .

As a doctor, if I’m ever stuck with nothing to do, I help the nurses and other doctors. I would grab that mop if I had to. Doctors should get over ourselves, if you have the free time but others are busy (and it’s not your break) then help others no matter what the job is, nothing is above you. We are all working for the same purpose so if we can help keep the wheels turning in anyway then we should.

Reminds me of my EP days when I scrubbed. We had impeccable turnover time and door to drape time. We even tracked it to compare other hospitals and we were most efficient. No matter, the EP doc would still watch us (arms crossed of course) through the control room window until we were ready.

the rooms with the fast turnovers have surgeons take the bed out so the circulator can help place monitors. if the circulator is placing monitors the anesthesiologist can start inducing. same with bringing the bed back in at the end of the case. after they dictate notes. they hang around due Ng turnover to answer questions. they can often help save a little time by telling the staff to skip opening certain things in the pack they don't use. likewise they can be there to ask for things that aren't in the pack so the circulator doesn't have to get them in the middle of the case and slow things down.

we have three urologists at my hospital. one does all that and the other two don't. it absolutely makes a difference as his average case times ate lower and the turnover times in his room (which surgeons think are beyond their control) are statistically significantly lower.

for you young surgeons out there. don't be prima Donnas. Help out by communicating your plan so the staff can get the right equipment and position it well. Touch base with anesthesia about anything out of the ordinary you may do that affects anesthesias plan. you DO have the power to make the day go faster, but you have to be a team player.

The head of dept of ophthalmology at my med school actually got around this by running 2 parallel tables in the same OT. While he was operating on one, the OT staff would be prepping the next patient on the other table. At the end of the surgery, all he needed to do for the next one was just turn around 180 degrees and he could start right away.

The OR is a stage, the table is their prop, the bright OR lights overhead are even brighter than those on Broadway, and they perform, perform, perform. I work with an ophthalmologist that actually opens her own cases and gives meds. It's wild, we clock a turn around of 4 minutes from the patient leaving to the next one entering and starting.

She tried to set up her own case once though because we were short a tech. I walk in to save the day and I've never seen a surgeon so glad to see a scrub. Like when a kid tries to cook supper but just makes a mess 😂

Exactly, I worked as a TSO through undergrad and most of medschool, I know exactly how long turnover should be taking because I did it more than a thousand times, I have no patience at all for deliberate manner it's often handled.

This is me waiting for my 22 year old daughter (and sometimes her boyfriend) to get ready to go somewhere with me or me with them. When I need to be somewhere, I'm usually ready to go - or am actually there at the location - at least 40 mins before time, usually an hour or more before. They just have to get every little detail of their appearances perfect, and every little chore they should have done hours before, completed last minute. It drives me insane. I'm all ready, makeup and hair done, with my "I have to go out" happy face on, (I'm a total homebody. I hate going out, even to the store. So it's an effort to get my level of positivity sufficiently high enough to meet my daughter's requirements) and she and her boyfriend are micromanaging every last tiny task that needs to be done including, but not limited to: getting their dog ready to go, stay home alone, or go to "grandma's" to be babysat, cleaning out their car so there's room for me to sit, talking to the neighbor just because, finding the debit card her boyfriend has lost somewhere in the house or car for the millionth time. And, on the way there: driving back to the last known location where he remembers seeing/using said debit card, putting gas in the car, deciding which store they want to stop at to get the thing they should have gotten the previous day, calling boyfriend's work to let them know he has been "unavoidably detained" and will be at least an hour late, calling grandma to make sure "Loaf" (their dog) is not suffering unduly from separation anxiety, waiting while "Loaf" is brought to the phone to listen to "Daddy" telling her how much he misses her and not to worry, he'll be back for her soon -(not likely)-, and yelling at me for yelling at them to hurry up or we'll be late to the designated place we're going. And fnally, upon arrival, searching for the perfect parking spot so we don't have to walk 500 ft to the door in nice weather, and then, just as we're finally ready to go in, meeting, just by chance, one of his sprawling familiy members in the parking lot and going through basically the same convo they had with "the neighbor" three hours ago but with the added time it takes to hear about their day, any family news that's going on, "how's the wife/girlfriend/dog/job doing - (except for "grandma" there are only boys in his family) -, and when we finally get ready to go in, finding out it's the wrong day/the place is closed/was never open in the first place and/or won't honor or alter their hours/reservations/movie time or other important detail just for my daughter and, more importantly, "the boyfriend", because they "had important stuff" that needed doing. Arggh! The frustration makes me want to tear my hair out, bite my nails, scream to the heaven's for them to 'for goodnes sake, manage your time more effectively!'

I used to work as the OR housekeeper. I would mop and sanitize everything. The goal was 10 minutes - 20 minutes depending on the surgery.
Ortho and hip replacements always took longer cause bl00d. Optometry was like boom boom boom.
And a full clean at the end of the day was supposed to be 45 minutes.

I'm an intern and, I kid you not, I once got sent to the OR to join one of the operators in motivating the OR staff to work faster by staring at them with my arms crossed. The turnover time was supposed to be 30 minutes and the surgeons said that it'll surely be shorter than that if there'll be more people putting pressure on the staff. Well, it didn't get any minute shorter despite the operator letting out some of the most dramatic sighs I've ever heard.

Since applying the "each dept as characters" idea to my own career, I've becoming increasingly aware that my job (housekeeping) is the service worker version of the surgeon.

This was me during a (minor) fire incident (I promise it was nothing LOL), and we had to evacuate for about 20 minutes. But.... what am I supposed to do for that time...? Wait around??? Crazy.

This reminds me of the documentary "Can Gerry Robinson Fix the NHS" where one of the surgeons had set it up so he'd tell the team to go prep the next patient while he finished up so that by the time he was done the next one was ready to go, such a great idea

you nailed it!!! As an O.R Nurse for many years, with a specialty in Ortho- only 1 surgeon took the MOP and helped with clean up!! Most of them went to the Charge Nurse to complain...
