All you do to any video(not sure about really old ones...) you simply add "&fmt=18" to the end of the URL/address and it will allow you to have higher quality. Once you add &fmt=18 you should see a link underneath the video that says something like "watch in standard/high quality". For me its automatically on high for other people it might not be.

I dont :P I upload it in perfect sounding quality but youtube decides to destroy it... and this is the result. The &fmt=18 thing is something youtube have created, not me :P

I dont think its because people still use dial-up, I think youtube just cant be bothered upgrading. They clearly have the power for HD(yes I said HD) videos yet have they done anything? No! All they've done is add a link to certain videos that meet certain requirements(which mine dont...), that allow you to choose between "Standard" and "Higher" quality for those of use with decent broadband or higher speeds... Even "higher quality" still has a very low audio quality and small video size.
