1. Musician, likely living in Scandinavia in the late 1980s/early 1990s and using an Amiga, "tracks" a song with a program like ProTracker or ScreamTracker or OktaMed or who knows what else, creating in the process a fully-functional song as small as 1/100th or potentially even 1/1000th the size of a "modern" (comparably long running time) .mp3.
2. Song goes into game/demo/other program, often without credits, as the credits for the music would be given in the game/demo/other program, rather than in the song file itself.
3. "Mister Lame Fuck Ripper" comes along with his "Hey Mega Lamer Ripper Program" and extracts the music from the game/demo/other program, and probably does nothing more sinister than uploads it to a local BBS so people can hear the music from the game/demo/other program without actually having to play the game/watch the demo/use the other program.
4. Years later, the Internet is awash with completely anonymous .mod/.okt/.med/.669/.etc files; later formats, like .s3m/.mtm/.xm/.it and others, are far less likely to be anonymous, as the habit of integrating credits into the sample-text was well-established by the time those programs appeared.
