Nice collection of labels you just stamped all over the writer. Investigate the following concept : "If you label me, you negate me".
You have also failed to explain the ways that his writing was informed.

I will try : he developed cynicism. He saw infighting and foolishness among the less-educated, people he'd spent time with for documentary reasons, or been forced to spend time with as a soldier. He also spent time with the better educated and when they promoted such friction and ignorance it was always because of some plan to get a better position. More power, more money etc. He also learned that wars are not won with bravery or idealism. They are simply won with money.

Orwell himself was not an extremist but had been to extremes in life. Books - indeed storytelling - always require drama to keep people interested. Lastly, all artists write from what they experience in life.

Orwell began writing the book. He saw a beautiful future, a writing career. He grasped at good health. It eluded him. He wrote on. Orwell realised he was going to die. The character in the book takes a pummelling and (as a person, though not literally), Winston dies. Soon after the completion of the book, Orwell himself dies. It does represent Totalitarianism if taken further than it was in Germany, for example. Could it be described as an extremist's work? Or more autobiographical?

It represents precisely the Totalitarianism used by Pol Pot. First, they tortured you, then after you'd declared other(s) to be a conspirator they killed you. More people were brought in and the cycle continued. Nearly a quarter of the people were killed in this way and it would not have stopped but for outside intervention. Every member of the population would have been killed. Is that too extreme? It actually happened.

Extremist is a label usually reserved to mean 'goes too far'. I believe that the book 1984 hits just the right spot at an extreme of experience. I don't think it goes too far.
