I read somewhere that the uniform tug, at least initially, wasn't so much a planned character trait but because of the uniform riding up and the actor needing to adjust it down. Possibly in this case it was a character thing though.

McCoy might have a little fun with it, but he's not going to be mean about it. It's like in Beyond, when an injured Spock goes on about how much he respects Bones, and Bones stops him there, wanting him to preserve his dignity.
