After self-releasing her first three records and gaining popularity in New York City's independent music scenes, particularly the anti-folk scene centered on New York City's East Village, Spektor signed with Sire Records in 2004 and began achieving greater mainstream recognition. After giving her third album a major label re-release, Sire released Spektor's fourth album, Begin to Hope, which achieved a Gold certification by the RIAA. Her following two albums, Far and What We Saw from the Cheap Seats, each debuted at number 3 on the Billboard 200. 2016's Remember Us to Life peaked at 23 on the Billboard 200.
Mayor Bill de Blasio proclaimed June 11, 2019, Regina Spektor Day in New York City. Spektor was also inducted into the Bronx Walk of Fame on May 18, 2019, by Borough President Rubén Díaz Jr.

How I wonder
The Madonna she tell the truth, no?
She's been saying "I'll have a baby"
"When he grow up he become a killer"
I'm just a soul whose intentions are good
Oh Lord, please don't let me be misunderstood
Oh, Marcello
Never thought I'd really love you but I do so
My headwaiter's head on your platter
Just say the word and
Salome can kiss my gutters
I'm just a soul whose intentions are good
Oh Lord, please don't let me be misunderstood

Regina Spektor's "Oh Marcello" is a mysterious and enigmatic song that can be interpreted in several ways, but primarily focuses on misplaced intentions that lead to undesirable actions with severe consequences. The first verse begins with the singer questioning Marcello about the truthfulness of Madonna's statements, which speak of having a baby who will eventually grow up and become a killer. The singer seems to ponder the idea that one's fate is predetermined and it may be too late to change the outcome, hence leading to the worry if Madonna's prophecy can be thwarted.
The second verse is the singer confessing their love for Marcello and stating that even if Marcello turns on them, they will ultimately forgive him. The reference to Salome, kissing their gutters, and the phrase "headwaiter's head on your platter" allude to the story of John the Baptist and perhaps, the possibility of a similar betrayal or punishment being received by the singer. The chorus, "I'm just a soul whose intentions are good, oh Lord, please don't let me be misunderstood" seemingly suggests that despite good intentions, the singer fears being misunderstood and having their actions misinterpreted.
The repetition of the chorus throughout the song reinforces the theme of misunderstandings leading to negative consequences. The song's references to Madonna and Marcello can be interpreted in several ways, but the impact of one's actions and intentions, especially on others, is a prevalent theme.
