The track "She's Gone Away" makes a reference to this song and because of the fact that "She's Gone Away" was written for Twin Peaks: The Return, it is strongly speculated that "Reptile" is inspired by the character of Laura Palmer.
"Reptile" marks the catharsis of The Downward Spiral in that the real, human side of the protagonist is comprehending his emotional damage. He is not dissociating through the "inhuman" side of himself (i.e. that which was present in "Eraser") and he is contemplating his relationship to the "Ruiner" that has been haunting him. Musically, the song is slow and aggressive with a lengthy distorted introduction before clean vocals enter. The song notably does not feature the distorted "inhuman" vocals present on the majority of the album’s tracks.
Characteristic of the band's mid-90s industrial rock sound, it opens with an eerie and quiet machine-like sound collage sampled from the film Leviathan, which transitions into an imitative musical composition. The structure, repetitive in nature, contains three distinct but similar sections, all driven by machine-like percussion loops and undulating rhythm guitar and synthesized bass. The choruses reflect a brighter mood with prominent melodic synthesizers and glimpses of guitar. The three sections are joined by two quiet instrumental breaks, a technique used later on "Even Deeper." The ascending synth melody from 5:13 to 5:20 seems to be a reference to or an interpolation of "Laura Palmer's Theme" from the television show Twin Peaks. During this same section, there is a looped sample of a woman saying, "Kirk, help?" from the film The Texas Chainsaw Massacre. The "gong" sound that appears midway through the track and after the bridge is also used near the end of "Hurt", but the pitch is changed.
The drums used are the same as on "Fell From Heaven" by Lead Into Gold (a Paul Barker side-project, and one whose music video for "Faster Than Light" features a cameo from Reznor.) This may be strictly coincidence, given that some of the same or similar instruments may have been used, or it could be an actual sample.

The lyrics of Nine Inch Nails's song Reptile embody a mix of grotesque imagery and desperation. This Track, Heresy, and Hurt are probably my favorites off of Downward Spiral, this track is literally so viscerally gorgeous and well constructed. The foundation rhythms like a cold mechanical heart beat, as our protagonist kneels before this reptilian goddess begging for his hedonistic prayers to be answered by her.

I understand it as a song about addiction- Like so many others people think are about sex or written about girls but seems really about fighting inner demons & addiction but it depends on your life how you interpret his music
